1Responsible person is determined by: - The director’s Board if Applicant is a company; - The partners, if Applicant is a partnership - The steering committee/board, if Applicant is a cooperative society - The owner, if Applicant is a sole proprietor

How do I apply for the service? |
Day 1: 1. Lodge application at counter. 2. If any information is missing, provide the required information.
*If the application is in order, the Ministry will instruct to collect certificate the following day. *If there are any issues with approval, Ministry will notify via phone and instruct to amend if applicable.
Day 2: 3. Collect original of the registration certificate from Ministry counter. |
